Steps:
1. For registration records, complete and sign Form DO-11A and include a written explanation of the reason for request.
2. If requesting a record that is not your own, complete Section C; NJ MVC states notarization or attorney acknowledgement is required.
3. For title history, use NJ MVC Certified Title Search resources and attach required identity/payment materials.
